Suicide Reporting System

NHS QIS receives reports on all suicides involving persons with a mental disorder.

We aim to improve the sharing of solutions and lessons learned from suicide reviews across NHSScotland.

Our Responsibilities  

NHS QIS is responsible for:

  • governance arrangements
  • ensuring there are appropriate mechanisms in place to review the information collected
  • any lessons learned are shared.

We are developing systems that will:

  • incorporate the reporting of incidents and near-misses
  • collate and analyse NHS board information while maximising learning and improving patient safety
  • ensure consistent suicide reviews across NHS boards.

Suicide Reporting

The suicide reporting system was transferred from the Mental Welfare Commission to NHS QIS in April 2008.

Please send all suicide reports of patients who have had contact with mental health services (notifications and reviews) to NHS QIS.
